Eligibility for Membership
Any person able to document his/her lineal descent from one or more of the following Mayflower passengers is eligible to apply for membership in the Societyof Mayflower Descendants.

The quickest way to demonstrate bloodline descent is through a prior application of someone who is/was related to you. The genealogists at Mayflower headquarters will be happy to review your lineage for you and let you know if there is an application already on file that has documented a significant part of your line. You can access their Preliminary Review Form at https://www.themayflowersociety.org/join/preliminary-review-form. The fee for this service is $25.00. Alternatively, if you already know of a relative who is/was a Mayflower Society member, our Arizona Preliminary Application has a space for you to note that. If he or she is/was an Arizona member, we have their application on file already. If the relative is/was not a member of the Arizona Mayflower Society you can obtain a copy of his or her approved application for $25 from the General Society of Mayflower Descendants, P.O. Box 3297, Plymouth, MA 02361. You can link an approved application to yourself through the addition of birth, marriage, and death records that document the generations between yourself and where the previously approved application left off.
Initial Adult Application
Please send the following THREE (3) items to the membership chair to start
the application process.
1. Print and complete the New
Member Preliminary Application. Make a copy for yourself.
2. Print and complete our Application
Lineage Review Form that shows your line of descent.
3. A processing fee check in the amount of $5 payable to Arizona Mayflower
Society.
Send these three (3) items to the Arizona Membership Chairman, Shanna
Gallo, 3443 E. Glenhaven Drive, Phoenix, AZ 85048.
The Arizona Membership Chairman will then send you a pink worksheet application
and instructions.
If your lineage connects with that of a former or current member of the
Mayflower Society (in any state) the application procedure often can be
shortened. Therefore it is important that you communicate with the state
Historian, Maura Mackowski after your lineage has been reviewed to determine
how much of the four-page pink application worksheet you really need to

Upon completion of the pink application worksheet send it to the Arizona
Historian, Maura Mackowski, 1022 W. Juanita Ave., Gilbert AZ 85233. The
Historian can work with you by USPS mail or via email, Dropbox, or Google
Drive. Please talk to her about both options FIRST before submitting
ANY documentation, paper or electronic, other than your pink application
worksheet.
Please do NOT send the full amount due for your application at this time.
The state Historian will look at your application and documentation first
and let you know whether it is likely to be approved by the General Society
at Plymouth and how much additional evidence, if any, needs to be obtained.
When the state Historian tells you that your application is ready to be
mailed to Plymouth and mails you the final paper application for your signature,
you would then send the Historian two checks - one for $75 (that will accompany
your application when sent to the General Society) and one for $57 (that
will remain with the Arizona Society, and which includes the first year's
dues). Those dues apply to the calendar year in which the applicant's membership
is finally approved by the General Society Historian, except that if such
date falls between October 1 and December 31, the dues will be allocated
to the following calendar year.
Junior Membership Application
A family member may sponsor for junior membership any individual less than 18 years of age who is of the same family and bloodline as the sponsor and has a prima facie proven lineage. An application form may be obtained here, or from the Junior Membership Chair, Diane Moore, 8054 E. Monte Avenue, Mesa, AZ 85209. The completed application form should be accompanied by a one-time fee of $25.00 and a copy of the junior's long-form birth certificate. The child will receive a Junior Membership Certificate, a birthday card each year and a November mailing about Mayflower history. Junior memberships expire at the age of 19, but a junior may apply for adult membership after the age of 18. If the application for adult membership meets the usual requirements and is submitted before the age of 19, the Arizona Mayflower Society will waive the processing fee and the dues for both State and General Society for that year; however the applicant will still pay the General Society application fee.
Junior Friends of the Arizona Mayflower Society
This is a new category approved at our fall 2013 meeting. Junior Friends include adopted and step children (less than the age of 18) of adult members of the Arizona Mayflower Society. This will allow children of blended families to feel part of the Mayflower family. The adult member may be a parent, grandparent, or other close family member, and will complete the Junior Friends Application Form, but will not need to provide birth certificate or any other documents since this is a special category and does not involve lineage to a Mayflower ancestor. There is a one-time fee of $25.00, and the child will receive a special "Junior Friends" certificate, a birthday card each year and a November mailing about Mayflower history. This is a special certificate and cannot be used in any way as proof of eligibility for adult membership. Annual Adult Dues
Annual dues are $32 (General Society $15 and the Arizona Society $17). Dues are payable upon receipt of billing in December each year.
